The City of Chicago's web site has links to a complete set of community maps in PDF files:
Chicago Community Maps. There are links to citywide maps with numbers or names plus maps of each community.
This is the best set of maps for defining the official community areas that I have seen. The major deficiency is that boundary streets are not labeled in too many cases. For example, Montrose is the divider between Lincoln Square and North Center but shows to centerline without labeling on both maps.
